HIPAA generally allows covered healthcare providers to share PHI for treatment purposes without a signed authorization. This can include coordinating care with another provider.
However, many experts recommend obtaining an ROI anyway, if possible, because it lets your clients specify exactly what sensitive information can be released and protects your practice with a clear, documented audit trail.
A valid authorization is required when PHI is disclosed for a purpose not otherwise permitted or required by the HIPAA Privacy Rule. For example, a client may authorize you to release records to an attorney or another third party.

Your authorization must include:
- A specific description of the information that may be used or disclosed
- Who is authorized to use or disclose the information
- Who may receive the information
- The purpose of the disclosure. You can use "at the request of the individual" if a client initiates the request and does not want to share more information
- An expiration date or expiration event
- The client's signature and date
- If a representative signs, a description of their authority to act for the client
- A statement explaining the client's right to revoke the authorization and how to do so
- A statement explaining whether treatment, payment, enrollment, or benefits can be conditioned on signing
- A statement explaining that the information may be redisclosed by the recipient and may no longer be protected by HIPAAIt's worth reviewing the Privacy Rule, which spells it out in detail.
